Our professional care managers advocate on your behalf and help solve problems. They make sure there is clear communication among family members, doctors, caregivers, and other healthcare providers. They ensure you receive the information and support you need to make informed healthcare decisions and bolster your independence and quality of life.

What Do Care Managers Do?

Care managers can support your or your loved ones in a variety of ways, including:

  • Creating an individualized plan for your care
  • Accompanying you to medical appointments and reviewing treatment options
  • Securing benefits and other financial assistance
  • Facilitating family discussions
  • Supporting your safety at home
  • Providing emotional support
  • Helping you define and communicate your healthcare priorities

How Do I Work with a Care Manager?

Whether you need a one-time consultation or ongoing care management, we work with you to create and implement an individualized care plan. We can provide expert consultations at your home, in residential facilities, or in our offices.

Seniors At Home has skilled, dedicated care managers across the greater Bay Area who can serve as your healthcare advocate. Our team includes gerontologists and social workers who see the big picture in complicated situations and make sure that nothing falls through the cracks. Meet our care managers.

Care Management through the Multipurpose Senior Services Program (MSSP) in Marin County

Funded through a grant from the California Department of Aging, MSSP is a state-wide program that enables frail seniors to live comfortably and safely in their own homes. Seniors At Home administers MSSP in Marin County. Residents of Marin aged 65 or older who meet certain qualifications may be eligible for care management services at no charge under MSSP.
