  • Stanford Medicine Transitions of Care—Coordination of services for patients after they are discharged from Stanford Hospital or from post-acute facility, to home.
  • Vivalon—Community services for Marin County seniors and people with disabilities.
  • Senior Villages are community-based membership organizations that empower older adults to live in their own homes as they age, by offering a network of high-quality resources, services, programs, and activities.
