Seniors At Home’s professional care managers work like a “concierge.” They advocate on your behalf, help solve problems, and communicate with physicians, hospitals, and family. Care Managers can coordinate all the aspects of care for our clients, navigate the healthcare system, and provide peace of mind to family members. Our team of care managers across the Bay Area includes gerontologists and social workers who see the big-picture in complicated situations and make sure that nothing falls through the cracks.
Kate Blank: San Francisco
Kate obtained her MSW from Arizona State University and is a Licensed Clinical Social Worker (LCSW). While in graduate school, she began working with survivors of domestic violence and passionately pursued that work for over 6 years. After moving to the Bay Area, Kate began working at a local hospital with individuals of all ages, from varying backgrounds, and with complex medical issues. She then joined the Seniors At Home team in 2015 and relishes the opportunity to work with her clients more closely, and to provide ongoing support and advocacy for them. Outside of work, Kate spends her time being active outdoors and spending time with friends, family, and her dog.
Sydney Griffin: Sonoma County
Sydney received her MA in Health Education from the California Institute of Integral Studies following her undergraduate studies at UC Berkeley. As a Certified Geriatric Care Manager, she has been providing care management services to the senior community of Sonoma County since 1998, and has happily been a member of the team at Seniors At Home for the last five years. Care management is problem-solving work, and Sydney finds meaningful satisfaction in working with elders and their families as they navigate the transitions of aging. Sydney enjoys spending time with family and exploring the many beautiful parks and beaches of Sonoma County.
Kimberly Kletter: Northern Peninsula
Kimberly earned a BA in Psychology and an MA in Gerontology, and served as the Director of Social Services at a nursing and rehab facility working with long-term residents and discharge planning. Kimberly joined the Seniors At Home care management team in San Francisco in 1999. For the past eight years, she has been based in Seniors At Home’s San Mateo office, working with individuals and families on crisis intervention, long-term planning, and coordinating comprehensive care. Kimberly particularly enjoys working with her many clients who are Holocaust survivors and providing them and their families with the support they need. Kimberly spends her free time with her family and her dog, playing sports, traveling, and volunteering at her local Lab Rescue.
Laura Sigura: Southern Peninsula
Laura Sigura earned her MSW from the University of Maryland and after seven years working in hospital discharge planning in various hospital settings, she began specializing in social services case management. Laura has always loved working with seniors in particular, and over the past eight years she has dedicated her work to creating personal plans of care for older adults. The greatest perk of care management for Laura is building deep, long-lasting relationships with her clients and their family members. Aside from care management, Laura’s three great loves in life are her dog, her kids, and dancing—not necessarily in that order!
